ENROLLMENT PROCESS The 2001 Hourly Health Care Enrollment process has been improved for the upcoming year in an effort to provide eligible members (employees, retirees, and surviving spouses) more flexibility and provide certain enrollment efficiencies. A summary of the timetable for the health care enrollment process is attached. What's Different? There is no specific enrollment period. Eligible members may enroll 24 hours a day, 365 days a year. However, a member may change their medical and/or dental plan only once during any 12-month period. If the member would like to make a change during 2001, it will be assumed that they have been enrolled in their present plan for 12 months. An automated telephone enrollment system will be implemented to increase member convenience. The telephone enrollment system will be open on Wednesday, January 3, 2001, and will remain open year round. A member may request additional health care information (i.e., Plan Benefit Summaries) through the automated telephone enrollment system. Summaries will be mailed within 3 to 5 working days. The automated telephone enrollment system will have the capability to read back and confirm a member's election during the enrollment process. A written confirmation statement of the elections will be generated by the automated telephone enrollment system and will be mailed to the member within 3 to 5 working days of the transaction. How does a member change their medical and/or dental plan? The member: Calls the automated telephone enrollment system at 800/333-7444.· Enters the member's Social Security Number. · Follows the prompt to change their medical and/or dental plan. · Enters the three-digit plan option code of their medical and/or dental plan selection. · Saves their elections. Note: The member may call back and change plans as many times as they wish through the last day of the month. However, only the last change made will take effect, and all previous changes will be cancelled. What if a member does not want to change their medical or dental plan? The member: DOES NOTHING.· Will have their present medical and dental plans continue; may change plans anytime throughout the year as long as there has been at least 12 months since their last change. Note: If the member would like to make a change during 2001, it will be assumed they have been enrolled in their present plan for at least 12 months. For all subsequent changes, at least 12 months must have elapsed since their last change. How does a member request health care plan information? The member: Calls the automated telephone enrollment system at 800/333-7444.· Enters their Social Security Number. · Follows the prompt to request health care plan information. Note: Information will be based on their present home address of record.When can members enroll? The automated telephone enrollment system will be available at 8:00 a.m. Eastern Standard Time on Wednesday, January 3, 2001, The automated telephone enrollment system will be open 24 hours a day / 365 days a year. What if the member does not enroll in January? Members no longer need to change their health care plans in January. Instead of a limited, fixed open enrollment period, eligible members may change medical or dental plans at any time during the plan year, provided at least 12 months have elapsed since their last change (except for eligible members' first change, if they desire to make a change during 2001). Plan transfers will be effective the 1"' day of the 2"d month following the month of enrollment. For trample, a plan transfer made in January will take effect March 1; a plan transfer made in February will take effect April 1, etc. How often can a member change their medical and/or dental plan? Eligible members may change their medical and/or dental plan once during any 12-monthperiod. Eligible members may change their medical and dental plans at any time during the plan year, as long as at least 12 months have elapsed since their last medical or dental plan change. However, the practice of allowing a change of plans if a member moves outside the service area of their plan will continue. These types of changes cannot be done through the automated telephone enrollment system. Active employees should call the NESC at 800/248- 4444, and retirees/surviving spouses should call the Hourly Retiree Servicing Center at 8001426-4610 to affect such changes. What if a member needs assistance? To assist members, a help line will be available Monday through Friday, 8:00 a.m. to 5:00p.m., Eastern Standard Time. A member will have the capability to transfer from the automated telephone enrollment system to a help line representative throughout the enrollment process. Company Address of Record Each year, many mailings are returned from the post office because of an "undeliverable"address. As in past years, we ask for your continued support in helping correct those addresses. If mail is returned to the Company as undeliverable, we will forward the mail to theUAW benefits representative for distribution to your members and asks you to encourage them in correcting the Company records. If a member has moved and receives a letter with plan options outside of their newhome area, they need to call the help line and speak to a representative for assistance. The local Hourly Personnel Office should be available to assist active employees inmaking the address change timely.
